Appointments
We know you have many choices when choosing a Podiatrist in Bloomington, IL, so we have made requesting an appointment a simple process via our Web site. If you cannot keep your appointment time, please call our office at least 60 minutes prior to your scheduled appointment time.  There may be a $25 fee for any appointment cancelled or rescheduled within 60 minutes of the scheduled time.  Additionally, there may be a $25 fee if you miss a scheduled appointment.  If you arrive late for an appointment, we may need to reschedule your appointment.  You will bear complete financial responsibility for any fee(s) incurred.   Repeated missed appointments may result in dismissal from our practice.

New Patients - Please allow an additional 15 minutes prior to your appointment to complete necesary paperwork. These documents are also available on our website for your convenience to download and complete prior to your visit.

Insurance
Our office accepts a variety of HMOs, PPOs, and other health plans. Please call our office to verify acceptance of your insurance carrier.

Payment Options
We glady accept Visa, Discover, MasterCard, checks, or cash.

Self pay patients
Payment is required at time of services. Flexible payment plans are available.
